Frequently asked questions

  • I QUALIFY! What's next?
You must register for a Department of Health (DOH) account (one per person). Click here to register. Be sure to have your ID or Patient/Caregiver/Grow Site Certification Forms, as applicable ready to upload on the DOH website. Designate your provider as Marie Bernadette Mendrun It will ask you to pay the State Application fee of $38.50 A FACE-TO-FACE appointment is REQUIRED if you are a new client or were certified by another provider. That's ok! Because our mobile provider will go to YOU. Be prepared to provide any information confirming your chronic condition.
  • What is a 329 card?
The HI Club for Wellness provides certification for medical cannabis in Hawaii. For a patient to use medical marijuana, they must obtain a 329 card, which the Department of Health issues after an appointment with a qualifying provider. Our expert team works with patients to help them understand the process and guide them through the steps to obtain a 329 card.
  • Do I qualify as a caregiver?
    - Patients may designate a caregiver to grow their medical cannabis or assist them in obtaining their medical cannabis from the dispensary if they are too ill to do so. Here's a breakdown of the definition of "primary caregiver" in bullet points: - A caregiver: * Must be 18 years or older. * Cannot be the qualifying patient themselves. * Cannot be the qualifying patient's physician or Advanced Practice Registered Nurse (APRN). * Must agree to be responsible for managing the qualifying patient's well-being regarding their medical cannabis use. *Special rule for minors or adults lacking legal capacity: The primary caregiver must be a parent, guardian, or person with legal custody. * A qualifying patient can only have one primary caregiver at any given time. *Exception for caregivers of multiple minors: If the primary caregiver is the parent, guardian, or legal custodian of more than one minor qualifying patient, they can be responsible for all of those minors. - Caregivers must be designated by the qualifying patient on the application and registered with the DOH in the Medical Cannabis Registry Program. - Caregivers may only grow medical cannabis for one patient at a time and must clearly indicate this in the electronic application. - Misleading or fraudulent information on the application may result in the revocation of the registration card and negate the ability to assert the medical use of cannabis as an affirmative defense to any prosecution involving cannabis. -If the qualifying patient decides to switch caregivers and informs the DOH, both the patient's and the original caregiver's cards will be invalidated. The patient will receive a new card along with the new caregiver, but the original caregiver will not be covered anymore.
